Best Practices for the Production of Cryoprecipitate

  • To maximize the amount of plasma in 1st stage cryoprecipitate, the AABB recommends at least 200 mL (205g) cell-free plasma.
  • Rapid freezing of 1st stage cryoprecipitate has been found to increase Factor VIII yield in Cryoprecipitate AHF; hence, use lower temperature freezer units for freezing in lieu of -18 °C freezer units.
  • Thaw 1st stage cryoprecipitate to “slushy consistency”. Do not over-thaw.1 Consider thawing similar volume 1st stage cryoprecipitate to better control thaw rate.
  • Chill centrifuge and centrifuge buckets and liners to 1-6 °C.
  • Load chilled centrifuge buckets with two 1st stage cryoprecipitate units per bucket inside refrigerated walk-in or using insulated gloves. Immediately, place loaded centrifuge buckets in chilled centrifuge.
  • Centrifuge at 1-6 °C using conditions to yield cell-free plasma.
  • Centrifuge using brake settings parameters that are moderate-hard to preserve cryoprecipitate pellet.
  • Remove spun 1st stage cryoprecipitate from the centrifuge and continue processing in batch sizes that allow the intermediate product to remain cold. Units should be handled gently. Consider using insulated gloves and/or placing on units on ice versus a room temperature countertop.
  • Leave adequate volume on single cryoprecipitate to produce “Wet Cryo” of > 15 mL. When pooling, consider producing 20 to 25 mL single cryoprecipitate products and maintain target volume range.

Best Practices for the Production of Pooled Cryoprecipitate

  • Load chilled centrifuge buckets with two 1st stage cryoprecipitate units per bucket inside refrigerated walk-in or using insulated gloves. Immediately, place loaded centrifuge buckets in chilled centrifuge.
  • Determine ABO identical 1st stage cryoprecipitate units prior to thawing.
  • Batch ABO identical 1st stage cryoprecipitate in processing steps.
  • Determine 1st stage cryoprecipitate volumes prior to thawing.
  • Batch similar volumes when thawing 1st stage cryoprecipitate units.
  • Sterile connect as close as possible to 6-to-1 connector to maximize recovery.
  • Pool single cryoprecipitate units simultaneously versus sequentially to reduce process time.
  • If flushing/rinsing tubing and bags, use insulated gloves or only touch the outside of the bag.
  • Use storage carton or bubble wrap final pooled product to reduce breakage.
